Wellington's Mayor says the city's bureaucrats have become too used to mediocrity.
The Government has released the findings of an independent Crown Review into the February disaster at the Moa Point wastewater plant.
It doesn't blame a single technical fault but finds significant gaps in accountability and risk management.
Andrew Little told Mike Hosking routines have been built around things not being done properly, and it's not an issue unique to the capital.
He says people get used to receiving reports stating what's wrong and nothing gets done, which becomes a repeating cycle.
